AC/AHSCP Completing an Assessment
This badge will equip the learner with knowledge and understanding of how to carry out a range of statutory assessments that are informed, ethical, holistic and co-produced.
This badge is one part of the Undertaking Assessment badge and will help a NQSW evidence their progression towards the following standards:
Standard 1 – Developing confidence and competence in the process of assessment using critical thinking skills and analysis to inform professional judgement and decision making.
Standard 2 – Collaborate effectively with service users and others to plan, implement and evaluate interventions.
Standard 3 – Positively promoting the wellbeing, support and protection of children and adults at risk of harm.
Standard 4 -Developing competence and confidence in the professional Social Work role and in managing complexity in professional decision making, drawing on research, evidence and best practice.
The criteria set below is an indicative list of sources of knowledge that can be utilised in order to achieve this badge, you may wish to use other sources.
- Listen to the podcast–on completing an assessment at http://www.helpfulsocialwork.com/3a-assessment. (approx. 30mins)
- Listen to the podcast-on analysis at http://www.helpfulsocialwork.com/4a-assessment-part-2 and reflect on what these means for your own practice.
- Complete a comprehensive assessment –outlining assessment of needs, risks and strengths which is signed off by a manager.
- Consider the assessment cycle: Assess, Plan, Intervene, Evaluate. Request feedback from your mentor, manager or a colleague on how you have applied the evidence base, the quality of your critical analysis and the appropriateness of your decision making and recommendation.
- Identify sources of research and up-to-date knowledge.
- Complete a self-assessment scaling tool on the skills you used whilst undertaking the assessment to help identify your future learning needs. (see also Supporting Toolkit – Council intranet)
- Review decision-making processes you must follow in your service area (see relevant Operational Instructions) and consider how your assessment fits into to this.

To successfully complete the badge learners must evidence their understanding of the key issues and demonstrate their application in practice by completing the activities listed below:
- Upload your reflections on the meaning of the podcasts for your practice
- Upload an anonymised assessment/ report that you have completed in your NQSW role
- Upload evidence of feedback received from your manager, mentor or peer on the quality and level of critical analysis within an assessment you have completed and what you have learnt from this.
- Upload evidence of feedback on how you have applied an evidence base, and the appropriateness your decision making and recommendation. Outline any ethical dilemmas and/or power differences you faced and how you resolved these.
- Upload evidence of a discussion about your use of the self-assessment tool with your mentor/ manager
We will accept evidence in audio, written or pictorial formats. Case examples can be in the format of anonymised records or reports.
